3. Fiduciary

Worksheet § 2. Each role defaults to the Employer (Plan Sponsor) — that's the most common arrangement. To name an individual instead, untick Same as Employer and enter their details.

Plan Administrator

The Plan Administrator is the ERISA "named fiduciary" by default — the person/entity legally responsible for running the Plan.
